Hepatitis C – häufig übersehen und unterschätzt
Der Begriff Hepatitis setzt sich zusammen aus „hepar“ (griechisch für Leber) und der lateinischen Endung "-itis" für Entzündung.

Das Hepatitis-C-Virus wurde erst Ende der 80er Jahre entdeckt. Doch obwohl Hepatitis C eine der am weitesten verbreiteten Krankheiten ist, ist sie selbst heute immer noch wenig bekannt und wird häufig übersehen. Experten gehen davon aus, dass weltweit bis zu 170 Millionen Menschen davon betroffen sind – aber nur die Hälfte weiß darüber Bescheid.

Die Virusinfektion wird durch infiziertes Blut übertragen, am häufigsten wurde der Erreger so in den 90ern durch Bluttransfusionen verbreitet. Der HC- Virus ist zwar auch in anderen Körperflüssigkeiten wie Muttermilch oder Sperma nachweisbar, jedoch ist hier eine Übertragung sehr selten. Der Erreger ist variabel und lässt sich heute in sechs Varianten aufteilen: Der Genotyp 1 ist in der häufigste und löst besonders oft chronische Erkrankungen aus. Anders als bei Hepatitis A und B gibt es gegen die sechs C-Varianten keine Impfung.

Die Inkubationszeit bei Hepatitis C beträgt zwischen 2 Wochen und einem halben Jahr. In manchen Fällen zeigt sich die akute Hepatitis anhand von Symptomen wie Durchfall, Abgeschlagenheit, Kopf- und Gliederschmerzen oder Gelbsucht. In 20% aller Fälle heilt die akute Infektion innerhalb eines halben Jahres aus, in den anderen Fällen wird sie chronisch. Es kann sein, dass die Krankheit jahrelang keine Symptome zeigt – daher wird sie häufig übersehen. Doch eine unbehandelte Hepatitis C kann auf Dauer zu Leberzirrhose oder Leberkrebs führen.

Um die fortschreitende Schädigung der Leber zu stoppen, ist es sehr wichtig, die Krankheit frühzeitig zu erkennen und zu therapieren. Seit dem 1.1.2001 besteht auch eine namentliche Meldepflicht für Hepatitis-C-Infektionen in Deutschland.


Neue Mittel im Kampf gegen Hepatitis C
– bessere Heilungschancen, kürzere Therapie


Mitte 2011 sprachen Ärzte von einer „neuen Ära“ in der Bekämpfung der Leberinfektion: Nach der US-Arzneimittelbehörde FDA hat Ende Juli auch die European Medicines Agency (EMA) den neuen Medikamenten Boceprevir und Telaprevir die Zulassung erteilt.

Bisher setzten die Mediziner ein Wirkstoffduo aus Interferon und Ribavirin ein: Interferon verstärkt die Immunantwort des Körpers, der Polymerasehemmer Ribavirin unterbindet die Vermehrung der Viren. Doch leider hat diese Behandlung häufig starke Nebenwirkungen – angefangen bei Fieber, und Kopfschmerzen bis hin zum fatigue-Syndrom oder Depressionen. Zudem wirkt diese Therapie nur bei 40% aller Patienten und dauert oft über ein Jahr.
Boceprevir und Telaprevir sind Proteasehemmer, sie unterbinden ebenfalls die Vermehrung der Viren, indem sie gezielt bestimmte Enzyme des Hepatitis C-Virus blockieren. Sie könenn nicht alleine als Therapie eingesetzt werden, sondern werden zusätzlich zu Interferon und Ribavirin eingenommen. Die Heilungschancen beim Genotyp 1 steigen hiermit auf 80% und die Therapiedauer kann auf ein halbes Jahr verkürzt werden. Ein weiterer Vorteil: Auch bei fortgeschrittenem Krankheitsverlauf kann die Dreier-Kombination eingesetzt werden, auch wenn die Hepatitis schon zu Fibrose oder Zirrhose der Leber geführt hat. Nachteile: Die Nebenwirkungen von Ribavirin – Blutarmut – könne verstärkt werden. Weitere Nebenwirkungen der neuen Medikamente sind Magen-Darm-Probleme, Geschmacksstörungen und Hautreaktionen. Ohne das Duo Interferon und Ribavirin dürfen die neuen Medikamente nicht eingenommen werden, da das Virus sonst mutireren und resistent werden könnte.

Bis zum Jahr 2015 erwarten Fachleute die Zulassung weiterer, ähnlicher Präparate, so genannter Direct Acting Antivirals (DAA). Diese DAA werden auch im Kampf gegen den HI-Virus eingestzt und zeigen dort Erfolge. Die große Hoffnung liegt darin, dass man irgendwann auf das Nebenwirkungs-starke Interferon verzichten kann.
